Verifying Your Coverage
If you’re unsure about coverage, please reach out to your insurance provider. We encourage you to confirm your mental health benefits and any out-of-pocket costs with your insurer.
Questions to Ask Your Insurance Provider
Is Empowered Life Counseling (Maronda Robertson or Tom Dubbels; 3788 55th Ave South, Fargo, ND) considered in-network with my plan?
Does my plan include outpatient mental health benefits?
What is my copay or coinsurance?
Do I have a deductible? If so, what is it, and has it been met?
Out-of-Network Coverage
If we are not in-network with your insurance plan, you may still be able to use your out-of-network benefits. Many PPO plans reimburse a portion of counseling costs, typically 60–90%, after you meet your deductible. If you choose to use out-of-network benefits, we can provide a Superbill (a detailed receipt) for you to submit to your insurance company. Reimbursement is determined entirely by your plan and is not guaranteed.
Self-Pay
We welcome self-pay clients, including those without insurance. Please contact us to ask about rates.

Good Faith Estimates
If you are paying out-of-pocket, you can request a Good Faith Estimate of your expected counseling costs, including session fees and related charges. This helps you plan and avoid surprise bills. Estimates are not a guarantee of final costs and may change based on your needs. To request one, contact us before your first session.
